Cuff Deflation During HFOV

چکیده
High frequency oscillatory ventilation takes advantage of unique principles of gas flow when respiratory frequencies exceed 3 Hz (180 breaths per minute). These gas flow principles enable the alveolar gas turnover at volumes below or equal to physiologic deadspace. One of these principles is call asymmetrical velocity profile. As Froese and Bryan described in their “State of the Art” article on high frequency ventilation: “Hazelton and Scherer have proposed that even in a straight tube, since the velocity profile in one direction is parabolic and flat in the reverse direction, there will be net convective transport of material. This mechanism may be particularly effective at bifurcations where the inspiratory velocity profile becomes skewed with the peak velocities near the inner wall, whereas the expiratory velocity profile is usually symmetric.”

Setting mean airway pressure during high-frequency oscillatory ventilation according to the static pressure--volume curve in surfactant-deficient lung injury: a computed tomography study.
BACKGROUND Numerous studies suggest setting positive end-expiratory pressure during conventional ventilation according to the static pressure-volume (P-V) curve, whereas data on how to adjust mean airway pressure (P(aw)) during high-frequency oscillatory ventilation (HFOV) are still scarce. The aims of the current study were to (1) examine the respiratory and hemodynamic effects of setting P(aw...
متن کاملHigh-frequency oscillatory ventilation is not superior to conventional mechanical ventilation in surfactant-treated rabbits with lung injury.
The aim of this study was to compare high-frequency oscillatory ventilation (HFOV) with conventional mechanical ventilation (CMV) with and without surfactant in the treatment of surfactant-deficient rabbits. A previously described saline lung lavage model of lung injury in adult rabbits was used. The efficacy of each therapy was assessed by evaluating gas exchange, lung deflation stability and ...
متن کاملCorrective measures for compromised oxygen delivery during endotracheal tube cuff deflation with high-frequency percussive ventilation.
OBJECTIVE To determine the effect of endotracheal-tube cuff deflation on airflow and F(IO2) during high-frequency percussive ventilation (HFPV), and explore methods of correcting the cuff-deflation-associated decrease in mean airway pressure and F(IO2) at the carina. METHODS Using a mechanical lung model in our respiratory research laboratory, we measured circuit pressure near the connection ...
متن کاملSimple Arm Tourniquet as an Adjunct to Double-Cuff Tourniquet in Intravenous Regional Anesthesia
BACKGROUND Intravenous Regional Anesthesia (IVRA) is a well-known technique for producing analgesia during surgical procedures in the extremities. However, the rapid onset of pain following the deflation of a double-cuff tourniquet during IVRA is a serious disadvantage, leading patient suffering. OBJECTIVES The aim of this study was to evaluate the clinical effectiveness of a pneumatic arm to...
متن کاملLaryngeal mask airway performance: effect of cuff deflation during anaesthesia.
We studied the effect of deflating the laryngeal mask airway (LMA) cuff in situ on recorded respiratory tidal ventilation in 30 spontaneously breathing anaesthetized patients. Another 26 patients were studied in whom the LMA cuff was undisturbed. Deflation of the cuff to a pressure of 22 mm Hg (below an estimated arteriolar perfusion pressure of the pharyngeal mucosa), by removing approximately...
متن کاملInspiratory limb carbon dioxide entrainment during high-frequency oscillatory ventilation: characterization in a mechanical test lung and swine model.
BACKGROUND High-frequency oscillatory ventilation (HFOV) has been utilized as a rescue oxygenation therapy in adults with ARDS over the last decade. The HFOV oscillating piston can generate negative pressure during the exhalation cycle, which has been termed active exhalation. We hypothesized that this characteristic of HFOV entrains CO(2) into the inspiratory limb of the circuit and increases...
